Our program wirelessly sends the information from a compatible medical device directly to your health file for remote review and monitoring by your provider. With that data, your care team can check on your health status and quickly make decisions on how to provide the best care moving forward, whether it be a change in a treatment plan or a medication adjustment. RPM allows providers to continue tracking healthcare data for patients once they are discharge

Is there a Co-pay associated with this service?

Yes. For Medicare beneficiaries, there is a 20% beneficiary copay. However, this copay may be covered if a patient has supplemental coverage or Medicaid. Private payers will vary and may or may not require a copay for RPM services.

 This means the cuff may be wrapped incorrectly, or the air plug is loose. 

Re-fasten the cuff around your arm and correctly insert the air tube plug. Then please measure again. If you continue to see this message, please reach out to your RPM Coordinator.

No. In case of a data transmission failure (E5 or E6), up to 60 measurements are saved on the device and will be sent when a successful connection is achieved.

    Please avoid touching the weight scale or the blood pressure monitor with water. Clean the items with a dry cloth or a slightly damp cloth to remove dirt. Do not attempt to clean the reusable cuff with water.
